In order to ensure that each staff member and participant are protected , Chavalos requires that personnel and participants wear face coverings when performing duties for Chavalos, except those participants for whom face coverings are not recommended, like young children under 2 years of age. Chavalos requires all staff, partners, visitors, donors, and volunteers to remain six feet apart, or six feet of distance, whenever possible, and abstain from physical contact, including shaking hands. Liability Waiver -- Please Check if you Agree I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Chavalos Danzas for Nicaragua. I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any claims of negligence arising as a result of such activity from which liability could accrue to Chavalos Danzas for Nicaragua, it’ officers, agents, employees, instructors, teachers, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Chavalos Danzas for Nicaragua”) now and in the future. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury.
